Diabetes PATH

Diabetes PATH (Personal Action Toward Health) is a 6-week workshop designed to help people with Diabetes improve their health and feel better. DPATH teaches people with Diabetes several tools for self-managing their health condition. People who attend learn how to face daily challenges of living with a chronic health condition while still having a healthy and meaningful lifestyle.

What’s in it for you?

    Diabetes PATH (Personal Action Toward Health) is designed for people with diabetes or pre-diabetes. Includes diabetes education and support in an interactive workshop style. People are taught effective tools to better self-manage their condition and its impact on their life. You choose your own goals! Caregivers and significant others are also encouraged to sign up and attend as active participants.
