Medicaid Waiver is a comprehensive, community based, in-home services program designed for adults age 65 and older and adults age 18 and older with a disability who are medically (nursing home level of care) and financially eligible.
The goal is to provide an individual with the choice to remain at home with services instead of going into a nursing home. The program is a supplement to family, friends, neighbors and other caregivers that make up the informal support system.
Medicaid Waiver services may include: adult day care, chore services, home delivered meals, community living services (personal care and homemaking), non-medical transportation and much more.
Care Management is a comprehensive community based program designed to assist frail adults age 60 and over with navigating the maze of health care and in home services. The goal is to provide the individual with choice and assist them in connecting with community resources and services that encourage safe, independent living at home.
