Client Story - Sandra Hamby
Sandra Hamby transitioned into the community after being in the nursing facility for one year. She was living with family after she could no longer take care of herself and eventually went to the nursing home when her family could no longer care for her.
While in the nursing home she was referred to R2AAA to see if she would qualify for services. A member of the Nursing Facility Transition (NFT) Program at R2AAA visited with her several times while in the nursing facility.
The R2AAA Housing Specialist met with Sandra several times during this process as Sandra was not able to return home with her family and needed housing in order to transition. A transition plan was developed using Person Centered Planning principles. The transition plan encompassed Sandra’s abilities, needs, medical status and financial limitations, informal support, needed medical equipment and household and grocery items needed.
The Housing Specialist continued to meet with Sandra to discuss available housing options and complete applications for housing. The Housing Specialist was able to secure an apartment for Sandra. Support Coordinators from the R2AAA NFT Program worked diligently to secure needed services, medical equipment and delivery of furniture to Sandra’s new apartment. Support Coordinators met Sandra at her new apartment when she arrived to welcome her home!
Sandra said that she hopes to have her grandkids over often to visit. She said that she is “so happy” to be out of the facility and hopes to “never return.” Sandra said, “I sure do appreciate what y’all have done for me.”
