MI Choice Waiver is a community based, in-home services program offering you the choice to remain independent and safe in your home and community. MI Choice Waiver is designed for adults 65 and older and adults 18 and older with a disability who are medically and financially eligible.

This program is a supplement to family, friends, neighbors and other caregivers that make up the informal support system.

MI Choice Waiver services may include:

  • Community Living Support (personal care and homemaking)
  • Support Coordination
  • Environmental Modifications
  • Adult Day Care
  • Respite for Caregivers
  • Transportation
  • PERS (Personal Emergency Response System)
  • Self-Determination
  • and much more!

The Tiered Care Services Program is a comprehensive, community based, in home program that allows you to remain independent and safe in your home and community. Tiered Care is designed to help those who are 60 years or older, medically complex or in need of supportive services in order to remain living in the community. There are not any income eligibility requirements for most Tiered Care services.
