Doing Business With Us

The Region 2 Area Agency on Aging administers the MI Choice Medicaid Waiver program in our service area of Jackson, Hillsdale, and Lenawee counties. We maintain a pool of quality providers to serve our program participants. When services are needed, supports coordinators will contact a provider from the list and request the services. Being a provider does not guarantee a specific volume of clients.

You Will Need To:

  • Meet the program requirements.
  • Fill out the application package.
  • Attend quarterly provider educational meetings.

Potential new providers must apply to become a part of the provider pool and include documentation showing they meet the program requirements. Interested potential providers should contact the Contract Manager and request an electronic version of the application package. This package provides detailed information about what is involved in serving as a provider and the required agreement forms. It also serves as an educational tool for potential providers who are not yet able to meet the requirements.

Compliance with the Conditions of Participation and Service Standards are monitored through provider reviews. Attendance at quarterly provider educational meetings is expected. Providers that demonstrate areas of non-compliance with the requirements are subject to probationary status and/or termination of their Agreement.
